About this notice
This prelaunch notice covers the OnlyMenHealth public website. The full patient-service privacy disclosures and treating provider’s notices will be presented before clinical intake begins.
Website information
The launch waitlist asks for your email address and consent to receive OnlyMenHealth launch and pricing updates. These submissions are stored in the OnlyMenHealth WordPress website through Elementor Forms, separate from the planned Bask patient platform. Form and hosting systems may also record technical submission information, such as submission time, page address, browser details and IP address. Authorized website administrators can access submissions. Do not submit health histories, test results, identity documents or clinical photographs.
The planned clinical experience
OnlyMenHealth is the public-facing brand. The planned patient experience uses Bask technology and our approved network of independent licensed clinicians. Clinical questionnaires, care-team messages and records belong in the designated patient platform. The treating provider’s privacy notice explains its handling of clinical records.
How website information is used
Waitlist information is used to record your request and contact you about launch and pricing. It does not establish a patient account or a clinician relationship and is not automatically transferred into clinical intake. Website hosting and form services process the information needed to operate and secure this feature. You can unsubscribe from launch emails. Information may also be disclosed where required by law.
Cookies and external services
Website features may use cookies or similar technologies. Browser settings let you restrict or clear cookies, which can affect account functions. Linked services have their own notices. Detailed disclosures for any patient-service integrations and marketing choices will accompany their activation.
Privacy questions and records
The patient privacy contact and secure request route will be listed before enrollment opens. This public website does not currently provide a patient-record request service. Existing records held by another provider should be requested directly from that provider. Please do not send sensitive documents to an unverified address.
Your choices
Depending on your location and applicable law, rights may include access, correction, deletion, a copy of information or limits on certain uses. Clinical records can be subject to retention duties and are not automatically deleted when a website account is removed. Service-specific notices will explain the applicable request process.
